CHANGE OF REGISTERED GP: JANUARY 2019
Dr Smy will be left the Practice on Friday 28 December 2018
With effect from 29 December 2018 all patients currently registered with Dr Smy will be reallocated to other GPs according to their surname as follows:
Patients: A-C: Dr Carter
Patients: D-F: Dr Smethurst
Patients: G-L: Dr Webb
Patients: M-W: Dr Smethurst
This is on a temporary basis only pending the recruitment of a new Partner in the New Year.
Unfortunately it is not possible to write to 1500 patients notifying them of this temporary change.
Dr Winnie Kwong, Dr John Darcy, Dr Jenny Willis, Dr Corinne Fletcher and Dr Tim McMinn will all be here running Locum sessions during January 2019 and February 2019.
Patients will be able to book an appointment with any of these GPs or with their own registered GP.
LAUNCH OF EVENING AND WEEKEND GP APPOINTMENTS
A new weekend and evening GP service has been commissioned through Alliance for Better Care (ABC) to provide additional access to primary care services. New GP Hubs will be operating in Mid Sussex which will offer pre-bookable GP appointments for patients registered with a practice in Mid Sussex. Weekday evening appointments are being offered from Dolphins Practice in Haywards Heath and from Cuckfield Practice. Saturday appointments are available in Burgess Hill. You can book an appointment by contacting Cowfold Surgery in the normal way during our opening hours. GPs working at the Hubs will be able to access your medical records. In order to book an appointment you will need to give your agreement to this at the time of booking.

To enable us to improve communication with our Patients and to ensure that you can read and understand the information we send to you please let us know if:
- You need Information in braille, large print or easy read
- You need a British Sign Language Interpreter or advocate
- We can support you to lip read or use a hearing aid or communication tool
- You have any other communication/information needs

A MINOR INJURIES UNIT IS AVAILABLE AT HORSHAM HOSPITAL AND IS OPEN:
MONDAY TO FRIDAY: 9.00AM TO 5.00PM
THERE IS ALSO AN URGENT TREATMENT CENTRE AT CRAWLEY HOSPITAL WHICH TREATS MOST INJURIES OR ILLNESSES WHICH ARE NOT LIFE THREATENING
THE CENTRE IS OPEN 24 HOURS A DAY, 7 DAYS A WEEK
